The audit trail of scope widenings for one endpoint

Every additive scope change this endpoint has had, newest first: the field, the value it replaced, the value it became, and when. A widen that changed nothing writes no row, so this is a record of changes rather than of calls (ADR-0049). Reaching it needs the same ownership as widening.
